ForwardABPath MOC - Pathology Update 2019
You must be registered for and attend the Mayo Clinic Pathology Update 2019 in order to take the post-course MOC Reflective Question Survey. Once the course has adjourned, you will be able to submit your answer to the MOC Reflective Question Survey by going to the Start tab and clicking the Start button.
Your response should be at least 4-5 sentences in length. You will have 30 days post-course to submit your survey response. The deadline to submit your response has been extended to Friday, April 26, 2019.

Conveniently located on the Mayo Clinic Hospital campus, the Residence Inn Phoenix Desert View at Mayo Clinic offers spacious suites, complimentary hot buffet breakfast, highspeed internet access, fitness facilities, swimming pool, a business center, and free parking. Check-in time starts at 3 p.m.; check-out time is noon.
A block of rooms, studio suites, at a special group rate of $260 (USD), per night, plus applicable state/local taxes, is available for course participants and guests. Group rates will apply three days prior to and after the course dates, based upon space and rate availability. Attendees are responsible for their own reservations. The standard hotel reservation cancellation policy will apply to individual reservations unless otherwise specified.
Travel

Special rates include unlimited mileage and are subject to availability. Advance reservations are recommended as cars are subject to availability; blackout dates may apply.

Cancellation and Refund Policy:
Request for cancellations must be submitted in writing to cme@mayo.edu. When cancelling a registration for a course 14 days or more before the course start date, a full refund (minus a $75 administrative fee) will be issued in the same form of payment the registration was received. No refunds are granted less than 14 days before the course start date.
